Regular visits to nursing homes are part of a bid to cut down admissions to hospital, for one CCG.
GPs from Bath CCG & North East Somerset CCG have been visiting nursing homes as part of the Local Enhanced Services (LESs).
The commissioning groups hope that having health issues spotted at an earlier stage will ease pressure on the local hospital.
CCG chairman Dr Ian Orpen said the visits are about giving people the treatment they need in “the right place and at the right time”.

CCG chairman Dr Ian Orpen said the visits are about giving people the treatment they need in “the right place and at the right time”.
He said: “Working in this way means GPs get to know their patients in nursing homes much better and they can spot any changes in the patient’s condition earlier.
“This often means they an be treated in the nursing home, rather than being sent to hospital.”
More care home residents now have an advanced care plan which specifies how and where they want to be treated.
Data from the pilot will be presented to the CCG’s Health and Wellbeing Board later this year.
