One of the biggest fears CCGs hold in entering the new NHS is freedom. Or the lack thereof in this case.

Will CCGs be allowed to be locally-focused organisations and awarded the trust to make their own choices for their local population? Or are they doomed to forever fall in line with the big NHS bosses determined to rule from the centre?

The NHS Alliance, NAPC and NHS Confederation have partnered to create a unified voice.

The NHS Alliance, National Association of Primary Care and NHS Confederation have partnered to form a single CCG membership body – NHS Clinical Commissioners.

While a full merger between the three bodies has not been proposed, it is a “possible outcome” in the distant future, NHS Alliance Chair Dr Michael Dixon told GP Business.

Scorecards that are to be introduced "imminently" in general practices across London to measure their "effectiveness" could "skew" outcomes, it is claimed.

Caroline Kerby, Joint-Lead of the NHS Alliance's Practice Manager Network, told GPB the 'Balance Scorecards' will test practices on 40 indicators including key performance management, patient access and patient feedback.

While NHS London is expected to roll out the measure "imminently", Kerby said other SHAs may choose to develop the scorecards at "different times and in different ways".

The Chair of the NHS Alliance will retire from his post once the clinical commissioning framework has been developed.

Speaking to GP Business, Dr Michael Dixon said he only had a couple of years left in his post before he would have to give way to "young blood".

He emphatically denied there are discussions being held on whether NHS Alliance will wholly merge with the National Association of Primary Care (NAPC).

CCGs should be able to decide on theirown composition and size, not Primary Care Trust clusters for Strategic HealthAuthorities, claims the National Association for Primary Care and the NHSAlliance – the two bodies that formed the coalition.

It is likely the NHS Alliance and National Association for Primary Care (NAPC) will form an 'interface organisation' to cement their aim to "counteract the dangers of centralism", it is claimed.

Michael Sobanja, Chief Executive of the NHS Alliance, claims the local focus of commissioning needs to be protected and preserved in the face of the National Commissioning Board's increasing powers.

"We are meeting soon to put the nuts and bolts on what we are trying to do," he said.

Clinical commissioning groups (CCGs) are in danger of becoming the bottom tier of the commissioning hierarchy, warns the NHS Alliance.

The representative organisation has called upon the government to ensure CCGs are fully involved in authorisation processes and modernisation plans.

The NHS Alliance recommends the authorisation process become a dialogue rather than an "arms length assessment" and that there should also be a dialogue in relation to the roles and responsibilities of the NHS Commissioning Board (NCB).
